Dikey Boyutu Düşük Tam Protez Kullanan Bireylerde Yeni Yapılacak Protezin Okluzal Dikey Boyutunun Sefalometrik Analiz Yöntemiyle Saptanması

GİRİŞ ve AMAÇ: Bu çalışmanın amacı; kafatasında bulunan iskeletsel anatomik özellikler ile okluzyondaki alt yüz yüksekliği arasında kuvvetli bir korelasyon olup olmadığını istatistiksel olarak araştırmak ve bu araştırmanın sonucunda geliştirilen regresyon formülü yardımıyla tam protezlerin okluzal dikey boyutunu nesnel ve yinelenebilen bir yöntemle saptamaktır.
YÖNTEM ve GEREÇLER: 21 hastada Sefalometrik analiz yöntemiyle belirlenen okluzal dikey boyut artışları 2 mm ile 16 mm olmak üzere ortalama 9 mm dir. Artışlar artikülatör kullanılarak yapılmıştır. Hastalar; klinik değerlendirme formu, Helkimo Anamnestik ve Klinik Disfonksiyon İndeksleri, Manyetik Rezonans Görüntüleme yöntemi ile değerlendirilmiştir.
BULGULAR: 13 kadın hastanın %100’ü, 8 erkek hastanın %62.5’i protezlerini kullanmaya devam etmektedir. 21 hastanın %85.7 si, protezlerinin çiğneme işlevinden rahattır. 21 tam protez hastasının %90.5’i yeni protezlerinin estetiğinden hoşnuttur. Helkimo Anamnestik ve Helkimo Klinik Disfonksiyon indekslerine göre dikey boyut artışından sonra stomatognatik sistem yakınmalarında istatistiksel olarak anlamlı bir iyileşme görülmüştür. MR Görüntüleme Yöntemine göre protezini kullanmaya devam eden 17 hastanın 16 sında kondil ile glenoid fossa eş merkezli yerleşmiştir, yumuşak ve sert dokuda herhangi bir patolojiye rastlanmamıştır.
TARTIŞMA ve SONUÇ: Bir kerede yükseltilen dikey boyutun temporomandibular eklem bölgesinde herhangi bir zarara yol açmadığı görülmüştür. Bu nedenle, işlevsel anatominin hastaya iade edilmesinin hem şifaya hem de arzu edilen bir estetik görüntüye neden olduğu düşünülmektedir.

Determination of the Occlusal Vertical Dimension of the New Prosthesis in Individuals using Complete Dentures with Low Vertical Dimension by Cephalometric Analysis Method

INTRODUCTION: The aim of this study was to investigate statistically whether there was a strong correlation between the skeletal anatomical features in the skull and the height of the lower face in occlusion, and to determine the occlusal vertical dimension of complete dentures with an objective and repeatable method.
METHODS: The occlusal vertical dimension increases determined by the cephalometric analysis method in 21 patients were 2 mm to 16 mm, with an average of 9 mm. Clinical examination, functional evaluation of the stomatognathic system with the Helkimo index were conducted with the dentures. Additionally, MRIs of the patients were obtained and evaluated.
RESULTS: 85% of the patients adapted to their new dentures immediately. 85.7% of the patients stated that they could eat comfortably, 90.5% of the patients explained that the appearance of their new dentures were better. Symptoms evaluated by Helkimo indexes were reduced.
DISCUSSION AND CONCLUSION: An average of 9 mm increase in the vertical dimension of occlusion gave a successful treatment outcome for most of the patients. With the construction of the new complete dentures, functions of the stomatognathic system were improved. Also, MRI evaluations revealed that for this study, increase in OVD did not cause any pathological conditions in the temporomandibular joint area.
